You must: 1) be a teen between the ages of 14 and 18 years of age -- youth with special needs are eligible up to age 21; 2) be a resident of the City of Chicago; 3) provide evidence of a Social Security number or Individual Tax Identification Number (ITIN). If you do not have these documents, in some cases, you may elect to audit the program (participate without receiving a stipend). 4) You must interview for the program and be accepted.
Yes, as long as you meet the program attendance requirements. You can also choose to participate in a Club program, which allows you to drop in when you are able.
Your school may have a specific grade participation policy. Please contact your school liaison to find out what the policies are.
If you would like to switch programs contact your school liaison or program staff. It is preferable that all program switches take place by the first week of a program or midterm to avoid stipend issues.
An ITIN is an Individual Taxpayer Identification Number issued by the Internal Revenue Service to people who do not have a Social Security Number. To obtain an ITIN, you need to fill out IRS Form W-7 and attach a valid federal income tax return. These forms can be accessed online at www.irs.gov or by calling 1-800-829-3676. You also must provide forms that prove identity and foreign status to the IRS. The IRS Web site suggests that if you need assistance in obtaining this number, visit an IRS Taxpayer Assistance Center (TAC). The Chicago Center is located at 230 South Dearborn Street and is open Monday-Friday, 8:30am-4:30pm.
Notify your instructor and liaison immediately. You should provide accurate documentation to avoid getting dropped from the program.

Yes, but you are only eligible to earn one stipend.
In January, any teen receiving an award through an After School Matters program will receive a tax form (either a W-2 or a 1099). The purpose of this form is to tell you the total amount of money that you received during the previous year and any taxes that were withheld, if applicable. That information is also reported to the IRS.
If you are a pre-apprentice, apprentice or advanced apprentice receiving a stipend, and no taxes are withheld, the total of all of your checks/deposits is reported on a 1099 form.
