Oh no, I didn’t mean to decline or I’ve changed my mind– what do I do?
If you accidentally declined, contact the program instructor ASAP and asked to be re-enrolled. You will receive another enrollment status email allowing you to confirm your placement.
I declined because I spoke with another After School Matters instructor about being in their program – what do I do?
Contact the instructor of the program you thought you were going to be enrolled in to confirm that they have you on their list. Now that you’ve declined your initial program placement, the other instructor can now enroll you in their program. Once they successfully add you to their program, you will receive a new enrollment status email.
How to find the name/contact information for ASM program instructors:
1. Log into your application using your application ID under Edit an Existing Application– https://youthreadychicago.cityspan.com/Web/sms/OnlineEnrollment/Summer2020/Application.asp
2. Click on Step 6: Confirm in the left-hand navigation
3. Click on either the PDF or HTML button for the Print Confirmation/Consent
4. The instructor names will be listed as the Main Contact for each of the programs
I accepted. What’s next?
Before July 6 you will receive an email containing login information to your new After School Matters email address. Once you log in you will see an email inviting you to join your program’s Google Classroom.
It says my parent or guardian needs to sign the Program Acknowledgement, Consent, and Release (PACR) agreement but they already did. How do they sign again?
Because we shifted to a remote learning model for this session, we updated the language in our Program Acknowledgement, Consent, and Release (PACR) agreement. All teens and parents/guardians will need to re-sign. On June 18th all parents and guardians were emailed with a link to electronically sign. If they did not receive that email, they can click on the link in your initial application confirmation email. If you do not have either email, please contact [email protected].
